DrTBarghouthi Posted December 6, 2020 Share Posted December 6, 2020 Dear community, This is a 48 year old gentleman with a NW 7 pattern. He underwent an FUE procedure almost 2 years ago. He has been on minoxidil and Low level light therapy since. Not keen on using finasteride ever. He has a significant FHx of NW7. In fact his sons are starting to develop MPHL at this stage. He was keen on trying to cover as much of the area as possible. We did 2 surgeries that were around 12 months apart. The first procedure addressed the frontal zone and Midscalp and the subsequent one was for the crown. Surgery 1: 3370 grafts of which 350 were refined singles hairs. Motorized sharp 0.85 mm punch Sapphire blades: 0.9mm and 1.3 mm Surgery 2: 12 months after: 1250 scalp grafts. We considered taking some beard grafts but he chose to keep that option for subsequent times if needed. Total: 4620 He continues to use minoxidil and LLLT. He uses Toppik occasionally on the crown , but was washed prior to coming for follow up photos. He may consider further surgery down the line or SMP for further density illusion. 2 Dr. Taleb Barghouthi approved and recommended on the Hair Transplant Network. I would say he can probably get around 1000 grafts from the scalp still, mainly from the sides and intermediate zones. His beard is also a good option. On the last pictures it looks like that there is a noticable "shadow" which is starting about 0,5cm behind the hairline!? We washed it before the photos but that might be the shadowing from it showing. He is considering SMP though with us. I will post photos of that if it happens/ additional transplant. You should encourage him to further push for a 3rd surgery using the last 1000 scalp grafts and then whatever number beard Grafts you deem necessary - I’d love to see the final result after a third pass. I think if any hairs in the donor or recipient are improved then that is a plus. I think donors improve on finasteride in some cases too. He is keen on a 3rd pass yes.
